New Patient Forms

Our new patient forms are available online to simplify your initial visit. These forms help us better understand your medical background and provide the best care possible. If you have any questions about our patient forms or your appointment please don’t hesitate to contact us.

For your first visit we ask that you fill out our New Patient Packet, review our Privacy Notice, and bring a valid government issued photo ID and insurance cards. Also, if you need your records transferred from another provider’s office, you will need to fill out the medical records request form.

Please arrive 30 minutes early to allow processing time, or we reserve the right to reschedule your visit. 

Medical Records and Pharmacy Refills

Every patient’s records are stored as electronic medical records allowing you and your healthcare team easy access to your medical information when required. For prescription refills, please have your pharmacist fax a request to 214-705-1379.

Insurance

We contract with many insurance plans to provide medical care to their members. At the time of your appointment, you will be responsible for your plan’s office visit co-pay, deductible, or other charges not covered by your insurance. When you request an appointment, please verify that our providers are on your plan. If so, we will file your insurance claims for you.
